Tufts Alumni Awards Nomination Form

The nomination period for the 2014 Alumni Awards is currently closed. The nomination period for the 2015 Alumni Awards will open in early autumn, 2014. For information, please contact Taylor Austin, Alumni Relations Office at 617-267-5786, or Taylor.Austin@tufts.edu.

Alumni, faculty, staff, and friends of Tufts are welcome to submit nominations for any of the award categories listed below.

The following must accompany this form to ensure that all candidates are judged equally:

  • Nominee’s curriculum vitae (CV) or resume
  • At least one letter of support from one of the nominee’s constituencies (alumni, administration, faculty, staff, or friend of Tufts) stating specific reasons for the nomination

Incomplete nominations will not be considered by the Awards Committee. Additional information (such as articles, letters of support, etc.) may also be submitted.

DEADLINE

Submit nomination forms and other pertinent information by Friday, November 15, 2013.

Award nominations are welcome throughout the year preceding the deadline.

Award Category

Please select the award that best reflects the nominee’s achievements and contributions. Nominees not selected will remain in the nomination pool for one additional year. After that time, the full nomination must be resubmitted.
