Tufts Alumni Awards Nomination Form
Alumni, faculty, staff, and friends of Tufts are welcome to submit nominations for any of the award categories listed below.
The following must accompany this form to ensure that all candidates are judged equally:
- Nominee’s curriculum vitae (CV) or resume
- At least one letter of support from one of the nominee’s constituencies (alumni, administration, faculty, staff, or friend of Tufts) stating specific reasons for the nomination
Incomplete nominations will not be considered by the Awards Committee. Additional information (such as articles, letters of support, etc.) may also be submitted.
Paper nominations may be returned in the following ways:

Submit nomination forms and other pertinent information by Friday, November 15, 2013.
Award nominations are welcome throughout the year preceding the deadline.
